Structural Pest Control and Pesticides - Sleep Products Licensing and Registration

Information about new applications for NC Bedding Manufacturers; new manufacturers are first time applicants. Companies under new ownership or companies that did not renew their license from the previous year must submit new applications. Submission must include:

• NC New Bedding Manufacturer Application
• Registration Number Request Form
• New manufacturers requesting assignment of a North Carolina registration number are given 30 days from license issuance date to have labels printed and sent to the Sleep Products Section Office. New manufacturers requesting use of a number assigned by another state should send a sample law label and a copy of the current license from that state is required. 
• Sample Law Label. Copies or faxes of the law label will not be accepted.

Information about renewal applications for a Bedding Manufacture Licenses (current and in good standing as of the last day of February of the current licensing year). Note: Manufacturers under new ownership or changes that resulted in assignment of a new federal identification number must file as new vendors. Submission must include:

• NC Bedding License Renewal Application
• If requesting use of a registration number assigned by another state, a copy of the current license from that state is required.
• Sample Law Label. Copies or faxes of the law label will not be accepted. The law label must be examined for fabric type and formatting.

Information about new applications for a Sanitizer License. Submission must include:

• NC New Vendor Sanitizer License Application
• Sample Law Label must be provided within 30 days from the license issuance date. Copies or faxes cannot be accepted. The law label must be examined for fabric type, color and formatting.

Information about renewal applications for a Sanitizer License (current and in good standing as of the last day of February of the current licensing year). Submission must include:

• NC Sanitizer License Renewal Application 
• Sample Law Label. Copies or faxes of the law label will not be accepted. The law label must be examined for fabric type and formatting.
