Complete the Diagnostic Sample Submission Form
- Use permanent ink to label boxes.
- Fill out all required fields on the diagnostic sample submission form. To submit more than four samples, use the diagnostic continuation page.
- To get accurate results and recommendations, you must provide details of crop history and any symptoms observed.
- Give each sample a unique identifier (sample ID) made up of five letters and/or numbers that will help you remember to which area the sample corresponds. Write this ID in the designated spaces on both the sample box and on the submission form.
Package Samples
- Do not tape the sample boxes.
- Package boxes tightly within a cardboard shipping container to lessen the potential of damage during shipping. Never send samples to the lab in padded mailing envelopes or boxes holding uneven layers of samples as the samples can shift, crumble, and get damaged.
- Place the Nematode Sample Submission Form on top of the nematode sample boxes.
- Label the outside of the shipping container with "Diagnostic Samples" or "Problem Samples" to alert the lab that the shipper contains time-sensitive samples.
Add stuffing if you are sending less than what the shipper holds to avoid shifting of samples and damage during transit.

- US Postal Service: 1040 Mail Service Center, Raleigh, NC 27699. Allow additional time for delivery.
By Courier: Many NCSU County Extension offices offer a courier service to deliver laboratory samples to the Agronomic Division. Contact your County Extension office to inquire if this service is available in your area.
In person: Samples may be dropped off at the Agronomic Division loading dock Monday-Friday between the hours of 7: 00 a.m. - 4:00 p.m., except N.C. State holidays.
