Completing the Diagnostic Sample Submission Form

  •  Complete the diagnostic sample submission form, filling out all required information as denoted on the form.   
  • Use permanent ink to fill out forms and label boxes.
  • To get accurate assay results and recommendations, you must provide details of crop history.
  • Give each sample a unique identifier (sample ID) made up of five letters and/or numbers that will help you remember which area the sample corresponds to ex:  GOOD, BAD, COT1, COT2, BEANS or LAWN. Write this ID in the designated spaces on both the sample box and on the submission form so these correspond as well.
  • Plan to ship samples, along with submission form(s) and the appropriate processing fee, so they will arrive as soon as possible.

How to Package and Ship Diagnostic Samples

 

Be sure to label the outside of the shipping container with "Diagnostic Samples" or "Problem Samples" to alert the lab that the shipper contains time-sensitive samples.

  1. Do not tape the sample boxes. 
  2. Package soil sample boxes tightly within a cardboard shipping container to lessen the potential of damage during shipping. Growers should never send soil nematode samples to the lab in padded mailing envelopes or boxes holding uneven layers of samples as the samples can shift, crumble, and get damaged.
  3. Place the Nematode Sample Submission- Diagnostic form on the top of the nematode sample boxes before closing the shipper.

 Add stuffing if you are sending less than what the shipper holds to avoid shifting of samples and damage during transit.

By Mail:

  • UPS or FedEx: 4300 Reedy Creek Rd, Raleigh, NC 27607
  • US Postal Service:  1040 Mail Service Center, Raleigh, NC 27699.  Allow additional time for delivery.

By Courier:  Many NCSU County Extension offices offer a courier service to deliver laboratory samples to the Agronomic Division.  Contact your County Extension office to inquire if this service is available in your area.

In person:  Samples may be dropped off at the Agronomic Division loading dock Monday-Friday between the hours of 7: 00 a.m. - 4:00 p.m., except N.C. State holidays.
