Procedures for Obtaining a Farmed Cervid Movement Permit
Fully complete the Farmed Cervid Movement Permit Application form and submit it to our office.

When submitting the application, enclose a letter of authorization or importation permit from the destination state if this is an out-of-state move. Please allow 5 business days for review, approval and issuance of the Farmed Cervid Movement Permit. If approved, you will be sent a signed copy of the Farmed Cervid Movement Permit. Please note that ensuring compliance with all import requirements for out-of-state movements is the responsibility of the NC producer or the veterinarian issuing the CVI for the NC producer.
Instructions for Completing the Farmed Cervid Movement Permit Application
Part I
Licensee name, address, phone number, and county: – List the name, license number, mailing address, county, city/state/zip, and phone number of the applicant. The applicant is the person who owns the cervid to be moved.
Facility Site Location – List the exact site location and address of where the cervid are located (to be transported from) if different from the owner’s address. License No. – List the license number which is located on the Farmed Cervid license wallet card.
License No. – List the license number which is located on the Farmed Cervid license wallet card.
Part II
Name/organization, address, county, and phone number: – List the name, county, address, phone number, city/state/zip of the individual/organization where the cervid(s) will be moved or exported to. Date – Date cervid will be moved.
Part III
Species – List the species of Cervidae being exported. Use the following key:
WT = white-tailed deer FD = fallow deer
AX = axis deer MD = mule deer
SK = sika deer MJ = Indian Muntjac deer
CR = caribou/reindeer EK = elk/red deer
Sex – List the sex of the animal(s) being exported. Use the following key:
M = male
F = female
Birth Year – List the year of birth of the cervid being transported.
Bangle Tag No – List the bangle ear tag number attached to each animal.
Button Tag No. – List the button ear tag number attached to each animal.
Certification – Signature is required for application approval and license validation
