New Manufacturer's Registration and License

New manufacturers are first time applicants. Companies under new ownership or a company that did not renew their license from the previous year must submit new applications. For further information, please review the Memo for New Applicants.

Please submit the following:

• New Manufacturer Bedding License Application Form;
• Request for Registration Number Form;
• A sample law tag; and
  • New manufacturers requesting assignment of a North Carolina registration number are given 30 days from license issuance date to have labels printed and sent to the Sleep Products Section Office.
  • New manufacturers requesting use of a number assigned by another state should send a sample law label and a copy of the current license from that state is required.
  • Copies or faxes of the law label will not be accepted.

Manufacturer Renewal License

Renewal licenses are issued to companies that are current and in good standing as of the last day of February in the current licensing year. (Manufacturers under new ownership or changes that resulted in assignment of a new federal identification number must file as new vendors).

Please submit the following:

• Renewal Manufacturer Bedding License Application Form. (Call 919-733-3556 with any questions)
• If requesting use of a registration number assigned by another state, a copy of the the current license from that state is required.
• A sample law label.
• Copies or faxes of the law label will not be accepted. The law label must be examined for fabric type and formatting.

New Sanitizer License

General Information about the Application for Sanitizer license

Please submit the following:

• New Vendor's Application for Sanitizer License and
• A sample law tag must be provided within 30 days from the license issuance date. Copies or faxes cannot be accepted. The law label must be examined for fabric type, color and formatting.

Renewal Sanitizer License

Issued to companies that are current and in good standing as of the last day of February of the current licensing year.

Please submit the following:
